Report an Adverse Event

If you have experienced or observed a safety concern or adverse event related to any Copper State Research product, please contact us immediately. We take all reports seriously and will acknowledge receipt within 24 hours.

For medical emergencies, call 911 or your local emergency services immediately.

How to Report

Email us at safety@copperstateresearch.com with the following information:

Product name and lot/batch number (if available)
Date the event occurred
Description of the event or concern
Your contact information for follow-up

Our Commitment

Copper State Research will acknowledge all adverse event reports within 24 hours of receipt. Serious adverse events will be forwarded to the U.S. Food and Drug Administration (FDA) within the statutory 15-day reporting window as required.
